SweetSpot is seeking a Clinical Customer Success Manager with a CDCES certification to play an integral role in the ongoing clinical support for our clinic partners utilizing our remote diabetes monitoring platform and service, SweetSpot. This position involves analyzing patient glucose data, providing clinical recommendations, and supporting healthcare practices in their adoption and continued use of SweetSpot. The ideal candidate will have extensive experience in diabetes care management, strong communication skills, and a passion for leveraging digital solutions to improve patient outcomes.
Responsibilities:
Clinical Responsibilities:
Independently analyze and interpret CGM and/or insulin pump reports for T1D/T2D patients through remote, asynchronous reviews.
Provide timely clinical recommendations to supervising clinicians and proper documentation via the practice customer's EMR system.
Conduct follow up calls to patients when needed, or to discuss their monthly data reviews and communicate any changes in their regimen as directed by their physician.
Provide diabetes self-management education during monthly remote reviews as appropriate.
Customer Success Responsibilities:
Contribute to the growth of the SweetSpot program for our clinic partners, ensuring quality ongoing support and communication of clinical success metrics.
Engage and communicate effectively with healthcare practice customers to understand their needs and convey program progress, challenges, and successes.
Collaborate closely with clinician partners to identify and resolve challenges relevant to the clinical workflows, developing efficient solutions as required.
Utilize our CRM system, PipeDrive, for efficient internal communication and task management.

Certified RN, RD, NP or PA with diabetes experience
SweetSpot partners with endocrinology practices to provide virtual CGM monitoring and enhanced patient care. SweetSpot combines a centralized software platform for managing diabetes device data, such as data from CGMs and insulin pumps, with wrap-around clinical support services. SweetSpot’s virtual team of Certified Diabetes Care and Education Specialists (CDCES) perform monthly CGM data reviews and coordinate with providers and patients to facilitate treatment plan changes. By actively managing and remotely reviewing CGM data between patient visits, we ensure patients receive timely treatment adjustments to improve glycemic control and patient outcomes. Additionally, SweetSpot’s automated capture of reimbursable care events optimizes CPT code utilization, unlocking new revenue streams for practices and making our partnerships both clinically effective and financially profitable.
